Non-profit service provider of home and community care for older Western Australians – Juniper - is currently conducting a survey to gain a better understanding of the care needs of residents living in the Shire of Gingin.
Juniper provides care plans unique to each person which may include:
- Clinical Support - such as nursing, physiotherapy, and occupational therapy.
- Home Support - such as domestic assistance, meal preparation, and personal care.
- Social Support - such as transport to appointments and companionship.
They are keen to understand how many community members currently have either a Home Care Package or Commonwealth Home Support Program. They are also interested to understand how many community members need support in their home but have yet to be assessed by the government’s Aged Care Assessment Team (ACAT) and would like to find out more.
The survey takes approximately 5-10 minutes to complete and can be accessed through the link below.
